By checking the box below, I confirm that I understand that employees/students must have a valid drug screen, alcohol test, and background check with them or in the SafetyPlusWeb system to take Evonik Contractor Orientation. If your students do not have these documents or if these documents are not current, then by checking the box below you are authorizing these tests to be run and their associated costs to be charged to your account. I also confirm that I understand that failure to identify non-readers or Spanish only employees/registrants will result in the employees/registrants being rejected on the day of orientation. Cancellation fees will apply. Cancellations must be sent into Safety Plus no later than 48 hours prior to the beginning of class on that day to avoid cancellation fees.

Employee Orientation will always occur at 7:30 am Central unless otherwise communicated by the Safety Plus Lab support team.

By completing and submitting this form, you acknowledge and understand that failure to identify non-readers or employees/registrants who only speak Spanish will result in the employees/registrants being rejected on the day of orientation.
