    SPW - Entry of Data

    This form is for uploading data to be processed by SafetyPlus.
  • IMPORTANT UPDATE: This form link has changed. Be sure to update any saved bookmarks you may have to this new link.

  • Injury Note

    For the source data to be keyed into the system the following must be provided:
    - Employee involved in incident - name must match their name in the system
    - Area/location must be listed - if not already an "area" in they system "other" will be used as the area
    - Body part must be listed (with right / left designated)
    - Injury Cause must be listed and match a "cause" already in the system (if not matching, closest match will be chosen)
    - Injury Type must be listed and match a "type" already in the system (if not matching, closest match will be chosen)
    - Incident Result must be listed and match - if not "I need help determining" will be used

    UPDATE CLAIMS INFO - you will need to update the claim as it progresses with notes etc... - and if the claim is recordable and you want it to show on your OSHA log you will need to complete page 4 of 6 on the Incident Wizard - to do this go into your SPW account and click on the incident grid and click edit button on the claim you wish to update

  • Vehicle Note

    For the source data to be keyed into the system the following must be provided:
    - Employee involved in incident - name must match their name in the system
    - Area/location must be listed - if not already an "area" in they system "other" will be used as the area
    - Description of the Incident
    - Supervisor in charge of the Vehicle or Area the incident occurred
    - Vehicle information Make / Model / Notes / Truck # etc...

  • Other Note

    For the source data to be keyed into the system the following must be provided:
    - Employee incident was reported to
    - Location or address of the incident
    - Description of the Incident / damage / Date of Incident

  • Training Note

    Requirements for source data entry to be accurately processed:
    -Employee/Student(s) name must match their name(s) in the system - we suggest you have them use name and ID #
    -Instructor's name must match an instructor already named in the system - if not, put note in notes section below that this will be a new instructor
    -Training topic must match exactly a topic that is already in the system (topic cannot differ at all - we will not make a determination of what topic it should be entered into - we will only enter exact matching data) use the notes section for communication of detailed information

  • Substance Abuse Note

    Rules for data entry to be completed:
    - Employee(s) tested - name(s) must match their name(s) in the system
    - Reason for testing - needs to match a reason already listed in the system
    - Test type needs to match a type that is already listed in the system
    - Put any "specifics" in the notes section below
    - Collectors name must match collector already in system - or you must put a "new" collector in the notes section
    - Result must match the results available in the system (we will put closest match if it does not match exactly)

  • Instructional Material Note

    Our support team can add your company's content and a quiz to a course in your SafetyPlusWeb system.
    - The file formats we can use for content are documents, slide shows, PDFs, and mp4 files.
    - The file names should match the name of the course in SafetyPlusWeb
    - Make sure to provide the answers to any quiz you’d like loaded into your system. If you do not provide a quiz, we will use our 2 general acknowledgment questions.

  • Employee List Update Note

    Employee List Updates include new hires, terminations, or current employee updates such as a change to the employee's supervisor or department.
    If you wish to upload an employee list file, please attach that using the available fields at the bottom of this page.
    If you are submitting information for one new hire, you can complete the fields below.
